What To Bring

On your first visit to our office, you will need to complete forms for health history and insurance before we can start treatment. We also have a variety of forms for you to fill out to familiarize yourself with our particular location. If you like, you may bring your health records from your previous dentist. Remember to also bring in proof of dental insurance if you plan on using those benefits.

What To Expect

Once the paperwork is complete, we can get you in for an initial exam with one of our dentists. Our associates will evaluate your dental and medical history before taking digital images of your teeth. This helps our dentists to look for signs of any issues, such as periodontal disease or jaw problems. With the information and images in hand, we will then discuss individualized treatment plans as necessary. This initial visit takes around one hour.
