Here at Oral Health Associates, we understand the trust it takes to choose a new dentist. Dr. Glime, Dr. Adam Koch, and Dr. Brian Koch are committed to making each patient’s visit as comfortable and stress-free as possible. They are happy to walk you through the steps of your visit, and if you have any questions, feel free to communicate with them or any of our friendly staff. Here is the essential information for your very first visit.
On your first visit to our office, you will need to complete forms for health history and insurance before we can start treatment. We also have a variety of forms for you to fill out to familiarize yourself with our particular location. If you like, you may bring your health records from your previous dentist. Remember to also bring in proof of dental insurance if you plan on using those benefits.
Once the paperwork is complete, we can get you in for an initial exam with one of our dentists. Our associates will evaluate your dental and medical history before taking digital images of your teeth. This helps our dentists to look for signs of any issues, such as periodontal disease or jaw problems. With the information and images in hand, we will then discuss individualized treatment plans as necessary. This initial visit takes around one hour.
If you have any questions regarding our policies, procedures, payment plans, or treatments, please do not hesitate to ask us. Our entire staff is happy and willing to provide the honest answers you are seeking. We believe open communication allows us to serve our patients better, so ask away!
We are excited to start seeing you at the Oral Health Associates office for Green Bay cosmetic dentistry or general dentistry. Please feel free to contact us ahead of time if you have any questions or concerns about your first visit. Call us at (920) 437-3376 to schedule an appointment or request one online. One of our friendly office staff members will be happy to assist in any way they can. We look forward to seeing your smile soon!

You may have noticed infection control has always been a top priority at our practice. Our established processes ensure you receive care that is both safe and comfortable. The COVID-19 pandemic is not the first contagious illness to have circulated our country, nor will it be the last. Over the lifetime of our practice we have successfully prevented the transmission of many infectious diseases such as HIV/AIDS, Swine flu, H1N1, MERS, SARS in addition to our annual “flu season.” Furthermore, hepatitis in its many forms, HPV, herpes, etc. are all part of our everyday lives as your dental care team.
In addition to our standard and universal precautions we want to tell you of additional infection control procedures we have implemented. We follow the daily updates and recommendations of the American Dental Association, the CDC, and OSHA. You may notice the following changes at your next appointment:
We look forward to seeing you again and are happy to answer any questions you have regarding the steps we take to ensure the safety of everyone in our office. Thank you for being our patient and we look forward to welcoming back our patients, neighbors, and friends.
